MEDICATION GUIDELINES
According to Tennessee State Law and School Board Policy Number 6.405, if a child needs to take medication at school, the following guidelines must be observed:
*ALL medications must be brought to school in it's ORIGINAL CONTAINER by a RESPONSIBLE ADULT. (Medications will be given by the nurse)
*The child's name must be on the container.
*No medication will be given to a child unless we have a SIGNED note from a parent or guardian with the following information:
CHILD'S FULL NAME
NAME OF MEDICINE
DOSAGE TO BE GIVEN
TIME TO BE GIVEN
PARENT OR GUARDIAN SIGNATURE
DATE
Since we must keep all notes for our records, it is much easier if you send a note on full size notebook or typing paper. Forms are available at the school for your convenience.
